JOB SUMMARY


Central Health is looking for a skilled Licensed Master Social Worker or Licensed Clinical Social Worker to join our Behavioral Health Services team to provide compassionate, inclusive, and trauma-informed care with the overall goal of reducing systemic barriers to mental health and substance use support, treatment and services. The person in this position is responsible for providing a holistic care approach that includes strengths-based screening, assessments, crisis intervention, patient psychoeducation and advocacy, clinical consultation, group facilitation as well as other relevant social work activities, such as, but not limited to, referral and connection to support services.

 

These services are provided in partnership with other Behavioral Health Services and Central Health team members. Program offerings may be provided in various settings and may include travel between Central Health’s Outpatient Specialty Clinics and other satellite locations. The person in this position should have an embodied commitment to anti-oppressive and trauma-informed care that fosters patient healing through a collaborative approach centered on empowerment, transformation and hope. This position reports directly to a licensed clinical social work supervisor.

 

If not already licensed as a clinical social worker, the Integrated Care Social Worker should be pursuing clinical hours for Licensed Clinical Social Work licensure. Supervision of these clinical hours may be provided by an
approved Central Health Licensed Clinical Social Work supervisor. Approval for supervision from the state licensing board must be approved within six months of hire. After completion of all supervised hours, the LMSW will be expected to pass the Association of Social Work Boards (ASWB) exam within six months.

Responsibilities

JOB FUNCTIONS

-Respond to mental health and substance use-related crises through the provision of trauma-informed crisis intervention, de-escalation, risk assessment, and other evidence-informed interventions to patients.
-Works closely with the patient’s interdisciplinary care team, provider and other licensed healthcare facility providers to provide care that is in the best interest of the patient and consistent with Central Health’s policies, mission, and goals.
-Assists the patient’s care team and/or directly facilitates patient related meetings focused on establishing additional care priorities, goals, and care planning.
-Complete risk screenings and assessments of the multi-dimensional and holistic biopsychosocial-spiritual needs, strengths, and goals to guide strengths-based treatment planning for diverse breadth of patients.
-Delivers effective evidence-informed and emerging best practice clinical treatments in individual and group settings.
-Provide strengths-based and culturally responsive psychoeducation to individuals and groups in areas including but not limited to health literacy and harm reduction strategies to support patient understanding and well-being.
-While under clinical supervision, provides trauma-informed, strengths-based and culturally affirming short-term therapeutic counseling for individuals, families and groups to support and enhance a patient’s overall wellbeing.
-May manage a caseload of patients to support continuity of care.
-Advocate with internal and external stakeholders for continued focus on the choices, values and preferences of the patient. Work to support safe and seamless continuity of care through the delivery of care coordination services, as needed for patients.
-Actively foster a safe, supportive and healing environment for all patients and their identified support systems.
-Support the Behavioral Health Services program’s continuous quality improvement and utilization management initiatives and activities to ensure high-quality standards of patient care.
-Attend and participate in multidisciplinary team meetings, scheduled staff meetings, supervision sessions, trainings, and other relevant meetings necessary for the clinical, administrative and operational functions of the Behavioral Health Services program.
-Complete all direct and indirect patient care documentation in accordance with professional standards, program procedures and relevant local, state and federal regulations.
-Maintain knowledge of community resources and other benefits necessary to refer patients for services and to advocate for patient needs, priorities and goals.
-Foster a collaborative and professional environment within the organization and with external community partners.
-Attend relevant professional development training to ensure the delivery of best clinical practices and maintain all licensure and/or certification requirements.
-Perform other duties as assigned.

Qualifications

MINIMUM EDUCATION: Master's Degree of Social Work from an accredited program

MINIMUM EXPERIENCE: A minimum of 1 year of clinical experience in mental healthcare

 

PREFERRED EXPERIENCE: 2 years of clinical experience in mental healthcare and substance use treatment

 

REQUIRED CERTIFICATIONS/LICENSURE: 

Active and unrestricted Licensed Master of Social Worker in the state of Texas (LMSW), with willingness to be under clinical supervision by a Licensed Clinical Social Worker Supervisor.

 

PREFERRED CERTIFICATIONS/LICENSURE:

Active and unrestricted LCSW in Texas
