Overview

The Disability Application Program Supervisor is responsible for supervising and directing the day-to-day operations of the Disability Application Program and the Disability Application Specialists. This position will develop and monitor program performance standards and referrals. This includes referrals for individuals experiencing or at imminent risk of homelessness, who wish to apply for federal disability benefits utilizing the SOAR model, as well as individuals who are housed and/or terminal. The Disability Application Program Supervisor and team Specialists are responsible for meeting clients where they are most comfortable, in their home, a shelter, nursing home, hospital, clinic, or other public location.

Responsibilities for this position include training team members, creating and documenting processes and referral pathways and representing Central Health in community forums and collaborations. The Disability Application Program Supervisor will work closely with the Transitions of Care and Bridge Program teams and will facilitate monthly case reviews. This position will also work closely with the Analytics and Epic teams to develop reporting dashboards to monitor caseloads and identify areas for continued improvement related to the management of program services.

Responsibilities

Essential Functions:

  • Manage the day-to-day operations of the Disability Application Program.
  • Serve as the subject matter expert in the area of Social Security Disability Benefits Application.
  • Create and maintain training materials and provide training to all new team members.
  • Create and maintain standardized documentation for program management.
  • Assign team caseloads, supervise work activities, and productivity.
  • Schedule and facilitate monthly case reviews with the Director of High-Risk Populations and/or other appropriate physicians and clinical team members.
  • Ensure team interactions with clients and other Central Health departments are timely and responsive.
  • Develop, recommend, document and implement changes to program processes and procedures.
  • Work with the Director of Operations, Special Populations to monitor staffing ratio and needs as department scope of work increases.
  • Work as an information source and escalation point to answer team member questions and provide instructions as needed.
  • Maintain a reduced caseload of high-risk clients
  • Work collaboratively with internal departments to educate them on the Disability Application Program and referral process.
  • Review and interpret reports on referral processes to help determine whether metrics are being met and develop and execute recommendations for necessary improvements

Knowledge, Skills and Abilities:

  • Solid comprehension and understanding of medical terminology and how various diagnoses apply to the Social Security Administration’s Listing of Impairments is also fundamental to this position.
  • Incorporates independent thinking, judgment and initiative when interfacing with physicians, providers, nurses, and other health care professionals as well as members, families, and/or caregivers.
  • Handles confidential and time sensitive materials including protected health information that is subject to HIPAA.
  • Utilizes computer to perform word processing and data entry, creating and maintain databases, conduct online research, format letters, reports and other information, and composes routine correspondence.
  • Working knowledge of Community agencies/resources, financial programs, i.e., TANF/SSI and familiarity with principles of cognitive growth and development
  • Ability to work with multidisciplinary staff and community partners

Qualifications

Minimum Education: Bachelor's Degree (higher degree accepted)

 

Minimum Experience: 
5 years Related experience, with at least two years working with clients in a clinical setting, non-profit or governmental entity assisting clients in enrolling in local, state or federal programs.
