JOB SUMMARY
We are seeking a highly skilled and motivated Ambulatory Clinical Pharmacist to join our multispecialty clinic, Transition of Care, or BRIDGE clinic team. The ideal candidate will be responsible for optimizing medication therapy management for a diverse patient population across various ambulatory settings which focuses on providing integrated care for patients transitioning from acute settings, and who thrives in a team-oriented environment. This includes working closely with healthcare providers to ensure safe, effective, and personalized medication use, while contributing to overall patient outcomes and the quality of care provided.
Key Responsibilities:
Medication Management:
o Conduct thorough medication reviews and manage complex drug therapies for patients across multiple specialties in an ambulatory setting, including patients in the BRIDGE clinic.
o Develop, implement, and regularly update individualized medication therapy management (MTM) plans tailored to patient-specific needs and health goals.
o Continuously monitor patient responses to therapies, making necessary adjustments to ensure optimal treatment outcomes.
Collaboration and Consultation:
o Work collaboratively with physicians, nurse practitioners, and other healthcare professionals to provide integrated and patient-centered care across the multispecialty clinic, Transition of Care, and BRIDGE clinic settings.
o Offer expert advice on medication selection, dosage adjustments, administration routes, and potential drug-drug interactions.
o Actively participate in multidisciplinary care team meetings and contribute to patient care planning and clinical decision-making processes.
Patient Education and Counseling:
o Provide in-depth education and counseling to patients and their families on medication management, including proper medication usage, understanding potential side effects, and developing strategies for medication adherence.
o Address patient concerns and provide clear, actionable information about medications and treatment regimens to improve patient outcomes.
Clinical Documentation and Compliance:
o Maintain accurate, detailed, and up-to-date documentation of medication therapies, clinical interventions, and patient interactions.
o Ensure compliance with federal and state regulations, institutional policies, and pharmacy practice standards.
Quality Improvement:
o Actively contributes to ongoing quality assurance and quality improvement initiatives, focusing on medication safety, medication use reviews, and clinical outcome assessments.
o Analyze medication utilization data to identify patterns, trends, and opportunities for improvement in patient care.
Research and Development:
o Stay informed about the latest developments in pharmacotherapy, clinical guidelines, and emerging treatments in ambulatory care, particularly within the BRIDGE clinic model.
o Participate in relevant clinical research activities and contribute to ongoing clinical trials to enhance evidence-based care.
MINIMUM EDUCATION:
• Doctor of Pharmacy
MINIMUM EXPERIENCE:
• 2 Years of clinical pharmacy practice experience under a collaborative practice agreement, including providing drug therapy management in an ambulatory care clinical setting
PREFERRED EXPERIENCE:
• One-year post-doctoral residency
• Multispecialty, Transition of Care, or BRIDGE clinic experience
• Past preceptorship of pharmacy learners (Ex: interns, residents)
REQUIRED CERTIFICATIONS/LICENSURE:
• Pharmacist license with the Texas State Board of Pharmacy
• BLS
• Board certification through the Board of Pharmacy Specialties preferred upon hire
