Overview

The Community Health Worker Lead (CHW Lead) is an expert and trusted member of the community, with an expert understanding of the patient culture, language, socio-economic status, and life experiences of the community served. The CHW Lead provides expert cultural mediation between healthcare, social services, and the community. The CHW Lead oversees workflows that supports best patient practice for CHW, CHW II, and CHW Specialist. The CHW Lead helps coordinate Central Health events for members and patients. The CHW Lead assists in hiring, training, and coaching new CHW team members and serves as a mentor to guide and direct. The CHW Supervisor plans community outreach programs, activities, and initiatives. The CHW Lead provides
expertise on variety of services including social services, clinical education, advocacy, and assistance with applying for health care coverage and resources. The CHW Lead will be responsible training preceptors, monitoring time sheets, and evaluations of students, and teaching CHW interns and entry-level CHWs. The CHW Lead functions as a team expert to provide services through informing, outreach, patient navigation, community health education, and the analysis of feedback and patient satisfaction. The CHW Lead may provide services through community outreach, the Central Health Navigation Center, Central Health Clinics, or other health care settings.

Responsibilities

Essential Functions:

  • Proficient in duties of CHW Specialist.
  • Manages the creation and implementation of community outreach programs, activities, and initiatives.
  • Assists in hiring, onboarding, training, and coaching of CHW I, CHW II, and CHW Specialist.
  • Develops and monitors staff productivity and provides constructive feedback to CHW I, CHW II, and CHW Specialists.
  • Coaches and mentors CHW Interns, CHW I, CHW II, and CHW Specialists.
  • Orients and assesses competence and ongoing performance of less experienced CHWs and communicates results with director.
  • Determines needs for community health related program needs.
  • Coordinates CHW educational calendar
  • Schedules and provides staffing for community outreach events.
  • Proactively researches and recommends opportunities for new partnerships and initiatives.
  • Develops workflows under the supervision of Direct Manager.
  • Identifies and collaborates with multiple stakeholders to increase access for patient population.
  • Analyzes CHW productivity and provides feedback to Community Health Worker Program Manager.
  • Assists directors/managers with performance reviews and performance improvement plans for CHW interns,
  • CHW I, CHW II, and CHW Specialists.

Knowledge, Skills and Abilities:

  • Expert knowledge of enterprise and community resources and the ability to provide referrals to patients/families as appropriate.
  • Expert ability to connect patients with community resources and make referrals to appropriate providers and external partners.
  • Expert ability to communicate verbally and nonverbally using coaching, motivational interviewing, and active listening skills.
  • Skilled in coaching task prioritization, flexibility, and willingness to learn new responsibilities and to work in a changing environment.
  • Expert ability to exhibit compassion, vulnerability, and empathy when working with employees and patients and advancing CH mission of health equity and DEI goals.
  • Expert ability to manage team members, projects, and tasks independently. 
  • Expert ability in providing person centered care and trauma-informed care that is inclusive and culturally competent.
  • Expert ability to teach community-based classes. 
  • Skilled in teaching and coaching CHW interns, CHW I, CHW II, and CHW Specialists in effective communication, time management, and case load.
  • Expert ability to present information in a public forum and facilitate respectful and informative discussions.
  • Expertly skilled in collaborating with internal and external partners to enhance patient access and outcomes.
  • Expert knowledge of community deficits and resources for high risk patients.
  • Working knowledge and experience with Microsoft Office Products.

Qualifications

Minimum Education: 

  • High School Diploma

Minimum Experience:

  • 8 years Has extensive experience as a Community Health Worker and is able to perform the duties of a Community Health Worker Specialist

Licenses/Certifications:

  • Community Health Worker Certification Upon Hire Required
