The Director of High-Risk Populations serves as the subject matter expert for high-risk populations including those experiencing homelessness, those needing respite and clinically and socially complex individuals. This physician will build and oversee teams that care for these patients. The Director is responsible for the development, clinical oversight and patient care of the Medical Bridge Program, which provides acute and chronic care in areas such as respite, diversional healthcare, transitional housing, mobile services, and street medicine. This individual will support the team through program development, strategic planning, and clinic operations, including optimizing care models and developing metrics that allow evaluation of the team and care. The Director will be responsible for direct patient care in areas previously named. The Director is responsible for partnershipformation to advance patient health and well-being. This position will serve/lead appropriate committees/forums ensuring that Central Health has a voice in directing the care of these critical populations. This position will also serve as a member of the Medical Executive Board Committees. The Director will lead and/or participate in the planning and implementation of clinical services and new initiatives with an aim to advance health equity by both improving and reducing disparities in clinical outcomes for Central Health's covered population. Develop and deliver effective public presentations to Central Health Board of Managers committees, board meetings, stakeholders and partners, and the public. This position will also serve in the supervision of others as assigned.

Essential Functions

Lead and/or participate in the development, deployment and management of effective and efficient clinical services to Central Health’s covered population through a variety of methods including contracted services, clinical services oversight, clinical support infrastructure development and oversight of direct service delivery
Collaborate with others to build systems of care for high-risk populations that provide adequate access to high quality care that meets or exceeds community clinical priorities and standards of care
Serve as a liaison to various stakeholders including partner agencies, community healthcare providers, governing bodies, elected officials, public engagements, media outlets and others as required

Education
M.D. or D.O. from an accredited educational institution. Required
