The Sr. Associate Director Nursing (ADON) – Transitions of Care (TOC) is responsible for overseeing and managing the Transitions of Care nursing programs including Case Management, Care at Home, Acute and Post Acute. Along with TOC Clinical and Operations Leaders, the Sr. ADON – TOC is responsible for supporting the design, implementation, management, and evaluation of transitional care nursing programs and other related key initiatives throughout the Central Health care continuum. The Sr. ADON – TOC coordinates quality improvement activities and is a key resource to guide policy and procedure development derived from the best evidence. The Sr. ADON - TOC provides direct report operational leadership to the Transitional Care nursing team.
This position will work as part of triad leadership team in making decisions that best support clinical staff, extended care team members and providers to ensure patients obtain access to care that is timely, effective, efficient, and safe.
