Overview

The Sr. Associate Director Nursing (ADON) – Transitions of Care (TOC) is responsible for overseeing and managing the Transitions of Care nursing programs including Case Management, Care at Home, Acute and Post Acute. Along with TOC Clinical and Operations Leaders, the Sr. ADON – TOC is responsible for supporting the design, implementation, management, and evaluation of transitional care nursing programs and other related key initiatives throughout the Central Health care continuum. The Sr. ADON – TOC coordinates quality improvement activities and is a key resource to guide policy and procedure development derived from the best evidence. The Sr. ADON - TOC provides direct report operational leadership to the Transitional Care nursing team.
This position will work as part of triad leadership team in making decisions that best support clinical staff, extended care team members and providers to ensure patients obtain access to care that is timely, effective, efficient, and safe. Responsibilities

Essential Functions:

 

  • Along with the Transitions of Care Clinical and Operations Leaders, develops and establishes nursing operational and strategic goals and objectives consistent with Central Health Transitions of Care Initiatives.
  • Formulates related objectives, goals, and strategies, collaboratively assesses new opportunities, and develops actionable tactical plans to meet growth and programmatic objectives for the Case Management, Care at Home, Acute and Post Acute programs.
  • Participates as an effective member of the site Triad management team to ensure the TOC teams support the overall operational and fiscal activities of the site to include staffing levels, budgets, financial and operating goals such as productivity and quality.
  • Facilitates cross- functional performance improvement teams. Evaluates the progress of programs or services by benchmarking best practices and identifying measurable outcomes.
  • Utilizes an evidence-based approach to the development of Transitions Care programs and includes review of literature and standards and integration into practice.
  • Provides clinical leadership and direct supervision for the Transitions of Care nurse leaders, including the Case Management, Care at Home, and Acute/Post Acute Associate Directors.
  • Assists in developing written policies and procedures and practice protocols pertinent to the Transitions of Care program.
  • Promotes the growth of staff in areas such as team processes, decision making, and clinical skills by identifying (with staff) areas of improvement, goal setting, and formal and informal learning.
  • Supervises assigned staff, including but not limited to, establishing work assignments and priorities, weekly review and adjustment of timecard, weekly review of time off requests, input for performance evaluations, monitoring orientation of new hires, and coaching and counseling staff in conjunction with Director of Nursing and HR.
  • Adheres to Texas Board of Nursing’s Nursing Practice Act requirements and to other governing agency regulations. Collaborates with the Central Health Compliance team providing insight and guidance on relevant procedures and policies.
  • Maintains knowledge of and, where applicable, compliance with regulatory or credentialing agencies. Develops and disseminates policies and protocols as appropriate to ensure uniform delivery of services. Provides oversight and interpretation of best practices, policies, procedures, and regulations to nurse leaders and other designated team members.
  • Evaluates and implements changes to medical service functions and performance in alignment with the company's mission, philosophy, objectives, and policies.
  • Other duties as assigned.

Qualifications

Minimum Qualifications:

 

Education:

Bachelor's Degree in Nursing

 

Work Experience:

  • 5 years of Case Management, patient navigation, or discharge planning for patients with multiple complex chronic diseases and special populations.
  • 5-10 years of experience of progressive management/leadership experience working in a healthcare environment.
  • Knowledge working with and leading teams with clinical quality improvement, workflow development, patient care coordination/care management, staff development, and patient education.
  • Demonstrated competence in program, policy, and standard operating procedure development
  • Demonstrated knowledge of Joint Commission standards, HIPAA regulations, Quadruple Aim, and Value Based Care
  • Experience with Epic and training or support for Epic end user programs

Licenses and Certifications:

 

  • RN - Registered Nurse - State Licensure and/or Compact State Licensure in the State of Texas 
  • BCLS - Basic Life Support through the American Heart Association
